If you inactivate your twitch account while logged in ps4, you’ll get high chance of error 0495ba16. because twitch app in the ps4 have no auto unlink process. so only way to fix that error is self disconnect twitch account against your psn account. It will work fine fixing other errors.
Too easy but hard to find if you are not get used to playstation system settings.
1. Enter Account manager
2. Select link with other Services
3. you can see linked twitch account. select and disconnect
That’s all. I think This way is solve most problems.
